Set up team forms

Clinic admins build the forms staff can send, and choose which team each one goes to.

Go to Clinic Settings → Team Forms, or tap Manage forms on the Team Forms page.

Before you start

A form goes to one or more groups, so you need at least one group, like "Managers" or "Front desk". Set groups up under People → Groups. See Set up groups.

Start from an example

The Start from an example tab has ready-made forms: Employee discount change, Time clock fix, Something is broken and Suggestion box. Tap one to add it. Each starts as a draft you can change before anyone sees it.

Build a form

  1. On the Forms tab, tap New form.
  2. Pick an icon, then fill in the Name and, if you like, When to use it (a line telling staff when to send it).
  3. Under Send to, pick the groups that should get it. Everyone in them gets a bell alert, and it waits on their team space until it's done.
  4. Add your questions under Questions. Type one and press Enter, or paste a list to add several at once.
  5. For each question, choose its Type, and turn on Required if it must be answered.
  6. Tap Save.

Question types

  • Short answer: one line, like a pet name or an invoice number
  • Long answer: a few lines, for details
  • Pick one: one choice from your list
  • Pick any: tick as many as apply
  • Yes / No
  • Date: picked from a calendar
  • Number: like hours or an amount
  • Just text (no answer): a note shown on the form, like "Managers reply within 2 working days"

For Pick one and Pick any, put each choice on its own line under Choices.

Publish it

A new form is a Draft until you turn on Staff can send it. To publish, it needs a name, at least one question and at least one team. Once it's Live, staff see it on the Team Forms page.

Turning Staff can send it off takes the form back to a draft without losing what's already been sent.

Change or retire a form

Edit a form any time and tap Save. Forms already sent keep the questions they were sent with.

To retire a form, tap Archive. It's hidden from staff, but everything sent with it stays. Find it under Archived and tap Restore to bring it back.

Note

Only the sender, the teams a form goes to and clinic admins can see what was sent. Put anything private, like pay questions, on a form that only goes to your managers.
