What Team Forms is for
Team Forms is for the requests staff send to a team inside the clinic, like an employee discount change, a time clock fix or a broken piece of equipment. Clinic admins build the forms. Staff fill one in, and the team it goes to gets an alert and works it until it's done.
Team Forms is not for clients. Forms your clients fill in live in Docs. See What Docs is for.
Where to find it
Team Forms sits under Logs in the Go to menu, as a tab next to your clinic's logs and the Drug Log. You can also fill one in from any page: tap the + in the header and choose Fill out team form.
The tabs
- Fill in a form: every form your clinic has published. Each card shows who it Goes to.
- For my teams: forms sent to a group you're in, with a count of open ones. Clinic admins see this tab as Sent to teams, with everything sent to any team.
- Sent by me: everything you've sent, and where each one is up to.
Use Search forms and answers to find one by its title, the sender or anything written in it.
How a form moves
Every form sent shows a status:
- New: nobody has picked it up yet
- Being handled: someone on the team said I’ve got it
- Done: someone marked it done, with an optional note for the sender
The sender gets a bell alert when it's done, and anyone can add a note along the way.
Who can see it
A sent form is private. Only the person who sent it, the people in the groups it went to, and clinic admins can see it or its notes.
Note
Who sent a form and when are saved automatically, so forms never need to ask for a name or date.
