Entries from your online forms
If your clinic shares its On the Way forms, clients and referring clinics can let you know they're coming without calling. Their submissions land on the board by themselves. Here's how to spot and handle them.
How to spot one
- In the logged column, a form icon appears instead of a teammate's picture. Hover over it to see who submitted it.
- The open entry shows a Client submitted or Referral form badge next to the status.
- At the bottom of the entry you'll see "Submitted via online form by" and the sender's name.
What's filled in
- Patient / Signalment, with the age if they gave one.
- Client name and Contact info (phone and email).
- Presenting complaint, from what's going on or the reason for referral.
- Planned date and ETA, if they gave them. The date is today otherwise.
- Status: your first status that doesn't auto-archive.
- Referring clinic: for referrals, the referring practice. A practice that isn't in your directory yet is added to it. For client forms, the pet's regular vet if they named one (otherwise Client).
Triage status is left empty for you to set.
Answers and files
Extra details and answers to your custom questions show in the entry's chat, marked "via online form". Files they uploaded (up to 10, 25MB each) are listed under Attachments; click one to open it.
Who's told
Everyone at your clinic gets a notification when a form comes in.
