Build a client form

Clinic admins can build online forms clients fill in from any device, such as intake, consent or boarding forms. Answers come back to Docs → Forms.
Add one of houndkit's forms
- Go to Clinic Settings → Docs → Forms.
- Under houndkit forms, click Add next to one, or Add all.
The form is copied to your clinic as a draft. Edit the questions to suit you, then publish it.
Build your own
- Click New form.
- Enter a Form title and, if you like, Intro / instructions shown at the top.
- Add fields. Types include Short answer, Paragraph, Dropdown (pick one), Checkboxes (pick any), Single checkbox (agree), Date, Email, Phone, Signature (typed name), Signature (draw) and Info text (no answer).
- For each field, write the question, add help text if needed, and tick Required if it must be answered.
- Click Save form.
The client's name, pet's name and email are always asked, so you don't need to add them.
Handy options
- Show only when: show a question only when an earlier answer matches, like "What did they eat?" only if "Has your pet eaten today?" is Yes.
- Add an "Other" choice with a free-type box on dropdowns and checkboxes.
- Short name for PIMS copy: a shorter label, such as "CPR/DNR," used when staff click Copy for PIMS.
- Bulk add / edit: type or paste the whole form as text, one question per line. End a line with
*to make it required. Lines starting with-become answer choices. Click Apply to field list to turn the text into fields.
Publish and share
- Click Publish next to the form.
- Its link shows in the list, as
houndkit.com/f/followed by your clinic and form name. - Click Copy link next to it, or staff can copy the link from Docs → Forms.
Unpublish stops the form taking new responses. Duplicate makes a draft copy to edit. Delete archives the form and its responses.
Other form settings
These apply to every form and live on the Settings sub-tab of Clinic Settings → Docs, under Client forms:
- Consent message: shown above Submit on every form. It saves when you click away from the box. Leave it empty for the standard wording.
- "Copy for PIMS" skips unanswered questions: when on, copied responses leave out questions the client skipped. It saves as soon as you flip it.
