Build sections and questions

Sections and questions are your audit checklist. Clinic admins build them in Clinic Settings → QA → Questions. Set up your roles first, since every question names who is responsible.

Want a head start? Click Library to open the houndkit question library. Tick the questions you want (ones you already have are marked) and click Add … questions. You'll set who's responsible on each afterward.

Add a section

  1. Type a name in New section at the bottom and click Add section.
  2. Click the pencil on the section to set its Title, What it holds (see below) and a Section SOP that shows at the top of the section on every case. Open Look to pick the icon and the Background and Icon colors.
  3. Click Save section.

What a section holds:

  • Questions: the normal list of questions.
  • Billing grid (charged vs correct): a built-in billing grid (charged vs correct charge, the difference, and who was at fault) instead of a question list.

Drag sections to change their order. Archive section (in the pencil panel) stops its questions appearing on new cases. Past cases keep them.

Add questions

  1. Click a section to open it.
  2. Type in Add a question and press Enter. You can paste a whole list, one question per line.
  3. Pick a type and press Enter or click Add.

Adding a single question opens it straight away so you can set who's responsible.

Question types

  • Error check: one thumbs-down. Mark it when the error is found.
  • Error list: related errors together, each option with its own responsible role.
  • Yes / No: "Was it done?" No is the error.
  • Result pills: thumbs down, fine or thumbs up, each answer with its own words. Left alone counts as fine.
  • Billing errors: the charge grid.
  • Note: free text captured on the case, never an error.

A Yes / No or Error check question can be switched over with Convert to result pills. Past answers move across.

Older questions may show a legacy type. They keep working, but new questions can't use those types.

Edit a question

Click a question to open it. Every change saves on its own, so there's no Save button:

  • Question: the wording auditors see. It saves when you click away.
  • Type and Section: change the type, or move it to another section. Audits that already marked it keep it.
  • Who's responsible: pick the roles whose errors this is. Marked errors default to whoever holds those roles on the case. If your clinic grades with points, set the points each role loses.
  • Audit types: untick audit types the question doesn't apply to.
  • Errors in the list (Error list) or Pills (Result pills): add, reorder and remove options, each with its own role (and points, if grading is on).
  • When it shows: limit it to certain Visit types, Species or an Age range. Yes / No and pill questions can be set to Must check, so Submit waits for an answer.
  • Help for auditors: a Note shown under the error when it's marked, for the auditor and in the staff member's own view. Also Saved feedback lines (one tap each on the case), a Related SOP (on an Error list, each option can name its own), and an Automatic to-do. Tick Give the responsible person a to-do when this is marked and type the instruction (for example, "Amend the record and re-scan the signed consent form"). Without text, nothing triggers.
  • Dates: Team focus until highlights the question for a while (up to 3 at a time), and Retire on stops it on new cases from that date.

Duplicate copies a question. Archive hides it from new reviews. Click Done to close it, or Next question to move on.

Note

Changing or archiving a question never rewrites finished audits. A case keeps every question and option it marked, even if it's since been retired.
